What contributes to excessive hair fall?

The stages of hair growth include the growing phase (anagen), the transition phase (categen), the resting phase (telegen), and the shedding phase (exogen). However, the pathogenesis of hair loss results in a shorter series of growth stages, as seen below.

The anagen phase is greatly impacted by excess dihydrotestosterone (DHT), thereby negatively affecting the exogen phase—the phase that is responsible for the natural shedding of hair from the scalp. Although losing 50 to 100 hairs per day during the exogen cycle is considered normal, losing too much hair may contribute to balding. 

Excessive hair loss refers to the androgenic hormone present in some hair follicles within the scalp, dihydrotestosterone (DHT). Oftentimes, with older age, hair follicles thin, causing them to fall out. However, in patients with androgenetic alopecia (AGA), excessive activation of DHT and testosterone (T) shorten the growth (anagen) phase of the hair follicle, resulting in hair loss. Male and female hair loss can often be attributed to an inheritance factor from either parent.

Hair loss may be caused by a number of other factors, including chemotherapy, pregnancy, and health conditions. 

Despite this, there are many myths present about the factors that cause hair loss. For example, many often think that the restriction of air or blood flow to the head, such as wearing hats or frequent hair washing, may cause hair loss. This is false!

Hair loss happens due to few main reasons. Genes, past medical conditions, hormonal changes & nutritional deficiencies. Hereditary hair loss, also known as male pattern baldness or androgenetic alopecia (AGA), is the most commonly experienced form of hair loss which usually occurs or worsens as you get older. The first signs of male pattern hair loss can be a bald spot or receding hairline.

Hair loss treatments include medication, therapy or a mix of both, but finding out what is causing your hair loss is the first step.

What is androgenetic alopecia (AGA)?

50% of males and females suffer from androgenetic alopecia (AGA).

AGA refers to the inherited, polygenic disorder often distinguished by a receding hairline or overall hair loss. The hair follicles are usually lost in an “M” shape for men.

 

Interestingly, however, the characteristic hair loss pattern in androgenetic alopecia means that hair from the occipital area of the head (the hair follicles on the back of one’s head between the ears) is resistant to falling out.

AGA studies have developed an astounding correlation between the endocrinologic pathways between AGA and benign prostatic hyperplasia (BPH), which is a male condition that causes a non-cancerous enlargement of the prostate gland.

In both mechanistic pathways, DHT in excess shortens the anagen phase of the hair cycle in particular hair follicles. This then causes the follicles to reduce significantly in size to then produce finer hair than normal. Over time, this fine hair eventually falls out, leading to the balding effects of AGA.

Hair loss can be improved with vitamin and mineral supplementation. Vitamins and minerals are important for normal cell growth and function and may contribute to hair loss when they are deficient. What is DHT (dihydrotestosterone)?

Testosterone (T), an essential male sex hormone often known for its production during puberty, is naturally reduced to dihydrotestosterone (DHT) via enzyme 5-α reductase (5AR) in the prostate gland, scalp, skin, and liver.

DHT is also a male sex hormone, however it is considered to be more powerful and influential in hair growth, especially on the scalp, than T. DHT receptors present on hair follicles, depending on one’s genetic sensitivity of these receptors, have the ability to miniaturize the hair follicles, shortening the growth phase of the hair cycle.
Therefore, the inhibition of any of the three components involved in the AGA mechanism that converts T to DHT may lead to successful restoration of hair follicles, which were damaged by excess DHT. Research studies show that the circulating level of DHT is reduced by 80% when the conversion of T to DHT is inhibited.
